Routine failure led to patient’s brain injury at Haukeland

A failure in routines led to a patient having a brain injury at Haukeland University Hospital.

 

One year earlier, a patient had been subjected to the same malpractice at the Intensive Department of Surgical Services Clinic(KSK).The patient received air from a bag of saline solution into the blood, which was transported to the brain according to NRK news.

The hospital implemented measures that would prevent the same malpractice being carried out at other departments.One year later,in April 2016,the same error occurred again in another department.

A patient in the lung department ended up with a brain injury,and had significantly reduced visual acuity,also reduced short-term memory,and reduced concentration.

‘’KSK updated its routines without the other department getting to know about it. It was because there was no place in the hospital for a procedure aimed at implementing new routines at other departments performing the same treatment’’ said Kathan Al-Azawy,Director of the Lung Department.

Managing Director, Eivind Hansen of Helge Bergen, said that the routines of KSK should have been implemented in the lung department.

“Here, we should have learned more quickly, and made sure that routine changes were implemented throughout the hospital” he said.
